  • Yes. With the participant's consent, Lifest can collaborate with Support Coordinators, nurses, allied health professionals, doctors, specialists and other providers so relevant recommendations translate into everyday support.

  • Yes. Complex and progressive conditions can change over time or following a hospital stay, new recommendation or move. Lifest can review everyday support with the participant and relevant members of their support network.

  • Yes. Participants, families, Support Coordinators, Specialist Support Coordinators, hospital teams and relevant health professionals can contact Lifest. An initial conversation helps us understand the participant's circumstances and determine the appropriate next step.
