Yes. With the participant's consent, Lifest can collaborate with Support Coordinators, nurses, allied health professionals, doctors, specialists and other providers so relevant recommendations translate into everyday support.

Yes. Complex and progressive conditions can change over time or following a hospital stay, new recommendation or move. Lifest can review everyday support with the participant and relevant members of their support network.

Yes. Participants, families, Support Coordinators, Specialist Support Coordinators, hospital teams and relevant health professionals can contact Lifest. An initial conversation helps us understand the participant's circumstances and determine the appropriate next step.
